Engineer Gary Farrow will present two case studies; the first the collapse of a port facility iron ore tripper structure, and the second the collapse of a temporary works bridge structure on a busy motorway.

Geoff Fletcher, FESA Committee member and forensic engineer will present on the March 2018 collapse of a Miami bridge. During construction of this innovative pedestrian bridge the 945t prefabricated major span collapsed catastrophically onto flowing road traffic, killing five motorists and one construction worker on the bridge at the time.

Speaker 1: 

Gary is a registered Chartered Professional Engineer in both the Mechanical and Structural Engineering areas of practice, and an Associate Director at AMOG Consulting.   He has structural engineering experience across a broad range of industry sectors and has detailed experience in forensic engineering with failed and collapsed structures from heavy duty engines, mining machinery, offshore structures, bridge structures, harbour installations, plant, crane, and other mechanical items. He has authored numerous expert reports in a number of legal proceedings and will discuss aspects of the structural analysis processes he has undertaken as an expert witness when conducting these investigations.  

Speaker 2: 

Geoff is an experienced applied civil engineer (FIEAust, CPEng), closely connected to the building industry. He has specialised in the construction products and services sector, particularly new product development, precast concrete, sales support & marketing, and in technical training. He has published and presented on a range of influential industry topics. As a civil engineer he is FIEAust, CPEng, EC(Vic), RPEQ.

In June 2019 the US Occupational Safety and Health Administration (OSHA) issued a forensic structural engineering report on the collapse, having investigated the cause and factors contributing to it. Key findings of failure lie with the Design Engineers in the design itself & during construction, with the independent Construction Engineer / Inspector, & with the Design-Build Contractor – all these so far notwithstanding any conclusions drawn by pending litigation. These failures extend further from hard engineering aspects into ignored contractual & regulatory checks set up to safeguard the design & construction processes. This presentation will focus on that report, and in particular the engineering process failures, with universal application across all disciplines.
